Request for Fire / Rescue Records (i.e., copies of Incident Reports, Fire Inspection Reports, Fire Code Violations, etc.,) visit WPB Public Records Request and complete a general request. Please ensure a detailed request, so that it can be properly routed. Billing statements, medical reports, and inquiries for third parties (i.e. law firms and attorneys) are not available through this office.
Request for Fire / Rescue Records (i.e., Medical Reports) attorney’s & third parties please submit request along with PHI Authorization form via ChartSwap using provider name of “City of West Palm Beach Fire Rescue” or mail to West Palm Beach Fire Department Attention: Record of Custodian located at 500 North Dixie Hwy West Palm Beach, FL 33401.
Request for an Itemized Bill, invoice or inquires related to Billing, Please submit request directly to Change Healthcare (Optum) through ChartSwap using a provider name of "City of West Palm Beach EMS" with an address of 7955 NW 12th St, ste 100 Doral, FL 33126 and records type Ambulance/EMS Billing.
